Please note: this form should be completed by college staff first. After a staff member completes their portion of the request, the student involved will be sent a confirmation notification to ask them to confirm they would like to move forward with the submitted appeal/exception request. If they agree, they will be asked to include their narrative, and the full submission will be sent to Undergraduate Studies Dean’s Office.
The following actions can be submitted on the form:
- Exception to Credit-by-Examination Policy 10-017: request to allow more than 45 credit-by-exam credits to count for student
- Exception to Admission to and Registration in Non-Degree Programs Policy 30-055 (link to policy): request to allow more than 14 non-degree seeking credits to transfer in and count for student
- Appeal college decision on Degree Progression and Completion Deadlines for Undergraduate Students Policy 10-505: appeal the college’s decision on the Degree Progression policy regarding extending enrollment or adding an additional academic offering
- Appeal college decision on Academic Integrity of Students Regulation 3.025: appeal the college’s decision on an Academic Disruption case
- Appeal college decision on ARC Petition: appeal the college’s decision on student’s ARC petition. Please note, all other steps of the ARC petition are handled in Archivum. This is just if the student is appealing to the institutional level.
- Request a posthumous or in memoriam degree according to Posthumous Degrees Policy 10-047.
