Graduate Advising

For assistance in navigating your degree program, 

  • Contact the Associate Chair for Graduate Affairs, Dr. Lee, for curricular questions.
  • The Graduate Coordinator, Anthony Inman, may be able to assist you in navigating the bureaucracy.
  • Students should also consult The Graduate Catalog (Note: each student may choose to graduate under the requirements of the catalog of the admission year to the program, or any susequent catalog).
  • For current students, the program Canvas page provides handbooks, more time sensitive information, as well as additional information about the program.

Forms

We have collected departmental forms and the most commonly used forms from the College and the Graduate School.  Students may need forms or information from the following:

Course Registration
Course Permit Used by department for independent study, graduate seminar, directed research, dissertation hours, and thesis hours.
Frequently used forms for the Graduate School
Graduate Studies Petition Used to add, drop or change hours for student course registration after the end of the first week of classes.
Graduate Course Transfer Form Used to transfer graduate credits. Credits must be transferred in student's first semester.
Qualifying and Specialty Exams
Evaluation of the Fundamental Qualifying Examination Used for academic assessment of the program. For examiners, not students.
Supervisory Committee Proposal for Specialty Qualifying Examination Must be submitted after passing two fundamental qualifying exams at the Ph.D. level. Must be signed by two examiners and the graduate program director. Students are responsible for this form.
Evaluation of Specialty Qualifying Examination Used for academic assessment of the program. For examiners, not students.
Master's: Exam Option
Apply to Graduate You must apply to graduate through Student Self-Service. Deadline varies:
Fall: Mid September; Spring: Early February; Summer: Early June. Before submitting this form
Master's: Thesis Option
Supervisory Committee Form — New Appointment Declares your committee. It must be made of at least 3 graduate faculty. External member is not required for thesis defense. Another form is available to change the committee. Must be submitted to the program coordinator.
Apply to Graduate You must apply to graduate through Student Self-Service. Deadline varies:
Fall: Mid September; Spring: Early February; Summer: Early June.
Thesis Defense Request Required to schedule a thesis defense. Due at least three weeks prior to defense. Must be submitted to the program coordinator.
Thesis Defense Announcement Work with Coordinator to schedule room and fill out announcement at least three weeks prior to defense.
Successful Defense Form (Masters) Unsigned form due to department three weeks prior to defense so that it can be given to committee chair. Student may wish to bring a copy to defense. Verifies that the final oral defense has been successfully completed and that thesis is ready to submit to the Graduate School pending revisions.
Complete ETD registration Ensures your name is in commencement booklet.
Thesis Defense Instructions Information about thesis defense. Bring to defense for chair.
Evaluation of Thesis by Advisor Used by department for academic assessment of the program.
Bring to defense for advisor. Must be submitted by the advisor to the program coordinator.
ETD Certificate of Approval Certifies a student's completion and approval of their thesis by their committee and department/college. Bring to Defense for advisor. Must include 1st page of plagiarism check. Students may submit to graduate schools at etd@grad.usf.edu. Deadline varies: Fall: Early November; Spring: Mid March; Summer: Late June.
Submit thesis to ProQuest Submit final committee-approved thesis in PDF. Same due date as ETD COA.
Doctoral Dissertation
Master's Along the Way Form Designed for students in the PhD program in Mathematics who do not yet have Master's degree in Mathematics. Must be submitted to the program coordinator before applying for the candidacy and after completion of the requirements of the Master's program in Mathematics. PhD students with concentration in Statistics that do not have Master's degree may apply for a concurrent degree.
Admission to Doctoral Candidacy Submitted to the Office of Graduate Studies following the successful completion of the specialty qualifying exam. Normally submitted together with the Supervisory Committee Form - New Appointment
Supervisory Committee Form — New Appointment Establishes dissertation committee that must be made of at least 4 graduate faculty, with at least one of them being external. Must be submitted to the program coordinator following the successful completion of the specialty qualifying exam. Normally submitted together with the Admission to Doctoral Candidacy Form. Another form is available to change the committee.
Supervisory Committee Form — Change Request Submitted to the College of Arts & Sciences to request changes to an established dissertation committee.

AMS Annual Survey Please help us and the profession by filling out the linked form and returning it to the department Graduate Coordinator (e-mail preferred) with your other paperwork.  After we forward this form, the American Mathematical Society will publish your name and dissertation title in its annual Survey of Doctorates Granted issue of the Notices.
